Guides to the confusing parts.
Written for families sorting this out at a kitchen table, and for staff who file prior auth all day.
Guide
How Bridge prior authorization actually moves
The Medicare GLP-1 Bridge is a demonstration claim, not a routine Part D fill. A licensed prescriber files the PA. The $50 does not create TrOOP credit.
ReadGuide
The $50 does not count toward TrOOP
Medicare GLP-1 Bridge payments sit outside Part D. They do not credit the deductible or true out-of-pocket (TrOOP). Extra Help does not apply.

One oral waits on an empty stomach. The other does not.
Foundayo has no food or water timing restrictions. The Wegovy pill requires an empty-stomach window before food, drink, or other orals. Both can be $50 on Bridge.

The $149 Wegovy pill 4 mg cash price ends 31 August 2026
Novo's 4 mg oral Wegovy cash price is $149 through 31 August 2026, then $199. Maintenance doses are often $299. Commercial-plus-card can still be about $25.

Bridge covers Zepbound KwikPen — not every Zepbound device
The Medicare GLP-1 Bridge lists Zepbound KwikPen only. Other presentations and NDCs are outside the demonstration. Confirm the device before the PA.

Four legal paths, one cheapest month
In 2026 the legal price options are commercial-plus-card (~$25), Medicare GLP-1 Bridge ($50), manufacturer cash ($149–$299), or Part D for covered indications. Telehealth bundles near $449 are usually worse.

The Wegovy pill is a timed swallow, not a breakfast vitamin
Oral Wegovy is taken on an empty stomach with a small amount of water, then you wait before food, drink, or other oral medicines. Foundayo does not use that window.

Who the demonstration will even look at
Bridge eligibility is Part D or MA-PD, no 2026 Part D GLP-1 fill, a matching BMI tier, a demonstration drug, and a PA. Covered Part D indications stay off Bridge.
ReadGuide
Extra Help does not apply to the Bridge $50
LIS / Extra Help reduces Part D cost-sharing. The Medicare GLP-1 Bridge is outside that subsidy. If a covered Part D indication exists, Extra Help may make Part D cheaper than $50.

Shop the 2027 formulary. Do not shop a vibe.
Medicare annual enrollment is 15 October–7 December 2026 for 2027 coverage. Bridge still exists in 2027; it ends 31 December 2027. Formulary and pharmacy network decide the year after.

Foundayo is a Bridge drug — if the rest of the file matches
Foundayo (orforglipron) is on the Medicare GLP-1 Bridge at $50 per month. It is an oral with no food or water timing rules. Eligibility, PA, and the 2027 sunset still apply.

You may be the clerk. You are not the beneficiary.
Adult children can assemble a Bridge or Part D checklist for a parent. They cannot use their own savings card, invent BMI, or enroll the parent in a plan from the kitchen table.

Other orals do not share the Wegovy pill's first sip
The Wegovy pill requires a wait before other oral medicines. Thyroid, blood-pressure, and diabetes tablets often already own that hour. Foundayo does not use the window.

31 December 2027 is a date, not a rumor
Medicare GLP-1 Bridge is scheduled to end 31 December 2027. The $50 month is not a permanent Part D benefit. Shop formularies before the last fill.

The visit goes faster when the file is already a file
A clinician-ready GLP-1 checklist is indication language, dated BMI, coverage path, product-device pair, morning-med collisions, and the TrOOP or 2027 notes that belong on that path.
