What was approved, and when
Foundayo (orforglipron) was FDA-approved 1 April 2026. It is a daily oral. The label does not impose food or water timing restrictions. That is the operational fact families keep asking us to repeat.
Approval is not coverage. Medicare Bridge listing is not a Part D formulary listing. Three different stamps.
The $50 path
On the demonstration (1 July 2026–31 December 2027), approved Bridge fills of Foundayo are $50 per month. The $50 does not count toward the Part D deductible or TrOOP. Extra Help does not apply.
A 2026 Part D GLP-1 fill generally blocks Bridge. Type 2 diabetes, moderate-to-severe OSA, or MASH on the prescription belong on Part D, not on this $50.
If Bridge is closed
LillyDirect cash for the lowest Foundayo dose is often about $149. Commercial insurance plus the brand card can be about $25 when the member is commercial and the plan covers the drug. Government programs cannot use that card.
A telehealth bundle near $449 is usually a poorer purchase than LillyDirect cash plus a local visit.
Why clinicians pick it on the checklist
When the intake says the empty-stomach window is unworkable, Foundayo is the oral that does not fight breakfast and morning tablets. Wegovy pill remains available on Bridge for people who can keep the window.
The checklist should still carry BMI date, indication language, contraindication review, and the 2027 cliff note. An easy swallow does not shorten the PA.
Device confusion to refuse
Foundayo is not Zepbound. Zepbound on Bridge is KwikPen only. Do not let a scheduler swap 'the Lilly one' without naming the product.
