This is a cash-list change. It is not a Medicare announcement and it does not alter the Bridge $50.
Who should care this month
Uninsured people, commercial members whose plan excludes weight-management GLP-1s, and anyone illegally offered a savings card on government coverage. Those households are on the cash price whether they like the date or not.
Medicare beneficiaries who are Bridge-eligible still face $50 if the PA is approved — the cash step is their backup, not their primary path.
Do not buy a bundle to 'lock' $149
Branded telehealth packages that include the visit and the drug often land near $449 a month. That number is usually worse than manufacturer cash at $149, $199, or $299 plus a local clinician visit.
A bundle does not freeze the 4 mg promotional price past 31 August. Read the cancellation and dose-escalation terms. Dose goes up; the invoice follows.
Other legal rungs
Commercial insurance plus the brand savings offer can be as low as about $25 when the plan covers the drug. The card excludes Medicare, Medicaid, TRICARE, and similar government programs. A $400 plan copay does not always compress to $25 — cards have caps.

What to put in the checklist
If cash is the path, the checklist should name the dose on the prescription, the posted cash price that week, and the empty-stomach routine so the clinician does not escalate a person who cannot swallow the window.
If the person has Part D and a covered indication (T2D, moderate-to-severe OSA, MASH), run Part D first. Cash is the residue after a real denial, not a shortcut around a coverable claim.
