Foundayo vs Wegovy pill: routine, cash, same Bridge copay

Both orals can be $50 on Medicare GLP-1 Bridge. Foundayo has no food or water timing rules. Wegovy pill needs an empty-stomach window. Cash ladders differ.

What each tablet is

Foundayo is orforglipron, FDA-approved 1 April 2026, a daily oral without food or water timing restrictions. Wegovy pill is oral semaglutide, launched January 2026, taken on an empty stomach with a small amount of water and a wait before food, drink, or other orals.

They are not the same molecule. They are not interchangeable at the pharmacy. A clinician picks one.

Bridge treats the copay the same

If the demonstration PA is approved, each is $50 per month through 31 December 2027. That $50 does not count toward TrOOP. Extra Help does not apply. A prior 2026 Part D GLP-1 generally blocks both.

Cash does not treat them the same

Foundayo lowest-dose cash via LillyDirect is often about $149. Wegovy pill 4 mg cash is $149 through 31 August 2026, then $199; maintenance is often $299 on NovoCare.

Commercial-plus-card can be about $25 for either when the plan covers the drug and the member is commercial. Government coverage excludes the card.

The practical fork

If the person cannot protect an empty-stomach window or already has a crowded morning pillbox, Foundayo is the oral that fits the day. If they can keep the window and the clinician prefers semaglutide, the Wegovy pill remains a Bridge option.
