What to ask your plan.
Formularies change by plan, employer, and year. Nobody can honestly promise you a yes from a web page — so these tell you exactly what to ask instead.
- Aetna: plan-specific coverage, not a national GLP-1 yes
- Cigna: commercial, Evernorth, and Medicare are different desks
- Humana: a large Medicare book, still plan-by-plan
- UnitedHealthcare: OptumRx lists are not one national answer
- Blue Cross Blue Shield is an association, not one formulary
- Kaiser: closed pharmacy, still not a national GLP-1 yes
- Anthem / Elevance: multi-state Blues, multi-list reality
- Molina: Medicaid-heavy, Medicare present, still plan-specific
- Wellcare: a Centene Medicare brand, not a Bridge enrollment desk
- Centene: parent company, many contracts, no single GLP-1 answer
- CVS Caremark is a PBM, not the insurer — and not Bridge
- Express Scripts is a PBM — coverage is the client's, not the warehouse
Why there is no yes/no here
Two people with the same insurance card can get different answers depending on their employer’s plan design and the diagnosis in the chart. Anyone showing you a definitive coverage table is guessing.
